What to expect
Initial Check-In
Arrive 15 minutes early to complete check-in and any remaining paperwork with our front-desk team.
Medical History & Exam
Your physician will review your medical history, current medications, and perform a focused cardiovascular exam.
EKG Test
Most first visits include an in-office electrocardiogram so your physician has a real-time view of your heart's rhythm.
Medical Team
You'll meet your cardiologist and clinical team, who will explain findings and answer every question along the way.
Next Steps
If further diagnostics or treatment are needed, we'll outline a plan together and schedule what comes next.
Referral & Check-Out
Any specialty referrals, prescriptions, and follow-up appointments are coordinated by our front desk before you leave.
What to bring
To ensure a smooth and productive visit, please bring the following:
- Photo ID and current insurance card(s)
- List of all current medications and supplements (with dosages)
- Records or test results from previous cardiologists or your primary care provider
- Implanted device cards (pacemaker, ICD, loop recorder) if applicable
- Completed New Patient and Health History forms
- Your questions — we encourage them
New Patient Forms
Please complete the following forms before your appointment. Bring printed copies with you or email them to our office in advance.

Insurance Information
We accept and bill many insurance plans, including:
- Medicare
- Most PPO Plans — Anthem, Blue Shield, Cigna, Aetna, United Healthcare, HealthNet
- HMO Plans utilizing the Sharp Community Medical Group Network
We also accept cash, personal checks, debit cards, and major credit cards (MasterCard, Visa, American Express, Discover).
Billing Policies
- All copays are due at the time of service.
- Self-pay patients are expected to pay for office services at the time of the appointment unless prior arrangements have been made.
- Monthly statements are sent for patient balances. Accounts with no response after two statements and a final notice may be placed with a collection agency.
- A $10 late fee applies to accounts sent to collections, and there is a $25 fee for returned checks.
